  • Welcome to the Leadership VIP Program Application.


    This application is designed to help us get to know you, your leadership goals, and your readiness to participate in this year’s program. You will be asked to provide basic information, short written responses, and details about your proposed Purpose Project.

    The application is divided into the following sections:

    • Applicant Information – Basic contact and employment details
    • Short-Answer Prompts – Your leadership motivation, goals, and experience
    • Purpose Project Proposal – Your initial idea for the capstone project
    • Commitment & Supervisor Support – Confirmation of your participation and organizational approval

    Please take your time completing each section. Most applicants spend about 30 minutes on this form.
    You may save your progress and return later before submitting.

  • Application Questions

    Please take a few moments to answer the following questions. These will serve as a baseline into your leadership journey during the program and help us understand your interest in joining the 2026 cohort. Be as open and honest as possible. You'll revisit these answers at the conclusion of the LVIP program. 
  • Purpose Project

    Please respond to the following questions in 150 words or fewer for each response.
  • As a part of the Leadership VIP Program, each Fellow will complete a Purpose Project — a capstone that aligns with their role, passion, or an area of need within their organization or community. This project is designed to apply leadership learning in a real-world context and create meaningful impact for both the Fellow and their community. 

    Please note: This section of the application is optional. Session one will include a project workshop with our program facilitator, mentors, and program director, who will work with you to outline your purpose project. This section does not determine your admission to the LVIP program.

    The Purpose Project should connect to your current role, align with your leadership goals, and create a positive impact on your organization, residents, clients, or team. Projects can be clinical, operational, educational, or culture-focused in nature.

    Some examples include:

    • Developing a staff mentorship or onboarding program to improve retention and support emerging leaders.
    • Creating a resident engagement initiative that enhances social connection or purpose for residents.
    • Implementing a quality improvement project (e.g., reducing hospital readmissions, improving medication management, or enhancing infection prevention).
    • Designing a wellness or mental health program for staff to promote resilience and reduce burnout.
    • Launching a family communication and education series to improve collaboration and trust.
    • Introducing technology or process improvements to streamline documentation, scheduling, or care coordination.
    • Building a cross-departmental leadership team to improve communication and decision-making within the community.
    • Developing a community outreach partnership (e.g., with schools, volunteer groups, or local nonprofits).

    Letter of Commitment

    I hereby commit to fully participating in the LeadingAge Indiana Leadership VIP Program. I understand that this leadership development program is designed to enhance my professional growth and leadership skills in my career.

    By signing this letter, I acknowledge and agree to the following:

    Active Participation: I commit to attending all program sessions, engaging in group activities, and completing any assignments or projects associated with the program.
    Commitment to Growth: I agree to be an active participant in my own learning, contribute thoughtfully to discussions, and challenge myself to grow both personally and professionally.
    Collaboration and Respect: I will collaborate with my cohort members, listen to diverse perspectives, and demonstrate respect for the experiences and contributions of others.
    Confidentiality: I understand that certain aspects of the program may involve sharing personal or professional experiences. I agree to maintain confidentiality and trust within the group to create a safe and open learning environment.
    Accountability: I recognize that my participation is integral to the success of the program. I commit to being accountable to my goals, the cohort, the mentors, and the program facilitators throughout the duration of the program.


    I understand that this program requires time, effort, and dedication, and I am committed to making it a priority in order to gain the most value from the experience.
