Medical Consent & Release
I authorize the staff of ArtiSonata Studio to administer basic first aid and seek emergency medical treatment for my child if needed during camp hours. If I cannot be reached, I permit my child to be transported to the nearest medical facility and treated as necessary by qualified professionals. I understand that I am responsible for any medical expenses incurred and release ArtiSonata Studio, its staff, and affiliates from all liability related to medical emergencies or injuries during camp participation.*

Photo & Media Consent
By enrolling in our summer camp program, I give permission for photographs and/or videos of my child to be taken during camp activities for the purpose of documentation, promotional materials, and social media use by ArtiSonata Studio. These images may be used on the studio’s website, printed flyers, or social media platforms.No names or identifying information will be published without additional written consent.*

Waiver and Release of Liability
While ArtiSonata Studio takes every reasonable precaution to ensure a safe and supportive learning environment, I understand that participation in lessons and activities may involve certain risks, including minor injuries, accidents, or allergic reactions. I acknowledge that my child’s involvement is voluntary and that I am responsible for notifying the studio of any relevant health concerns.*

I agree to release ArtiSonata Studio, its staff, and instructors from any liability for injuries, losses, or damages that may occur during lessons or while on studio premises. I accept full responsibility for my child’s participation and understand that the studio is not liable for unforeseen incidents.
