By signing below, I understand and agree to the following:

1. I am voluntarily participating in the Halloween Party hosted by Poblacion Provisions on October 31, 2025.

2. I acknowledge that participation in this event may involve certain risks, including but not limited to slips, falls, costume-related accidents, or food allergies.

3. I understand and agree that children aged 7 years old and below must be accompanied and supervised by a parent or legal guardian at all times during the event. I accept full responsibility for the supervision and safety of the child(ren) I bring to the event.

4. I release and discharge Poblacion Provisions, its organizers, volunteers, and venue staff from any and all liability for injuries, illness, losses, or damages that may occur during or as a result of this event.

5. I consent to the use of any photos or videos taken during the event for promotional, social media, or marketing purposes by Poblacion Provisions.
