    Nominations must be submitted by 31st January 2026. Individuals who have previously won an award are ineligible to be nominated for the same category of award again and ineligible for another award category for a period of 5 years. Please complete a separate form for each nominee (ensuring their name is clearly written and spelt correctly) and submit it together with any supporting materials as attachments. Since the initial shortlisting process is down to the quality of the nomination form, please read the guidance for the selected category and complete the form as thoroughly as possible.     • The following awards will be announced during the Awards evening, chosen by the Judges from the categories listed above. Please refrain from selecting any of the categories below:  -The Craig Thomas Lifetime Achievement Award: Presented by Care Forum Wales, this award honours an individual who has made an exceptional contribution to the social care sector in Wales.  -The Spirit of Care Award: Chosen by the judging panel, this award goes to the nominee whose work best embodies the core ethos and values of social care.  -The Dignity in Care Award: Part of the Dignity & Respect in Care Programme for Wales, this award celebrates finalists who have demonstrated a strong commitment to embedding dignity in every aspect of their practice and have particularly impressed the judges with their dedication and practical approach.
    • Nominee's Professional Qualifications and (if applicable) Professional Registration (Nominations will not be considered if you do not provide the nominee's Professional Registration Number(s) if required for their role in the sector 
    • You MUST include specific examples of the nominee's work that highlight their EXCEPTIONAL CONTRIBUTION that inspired you to nominate them.

      When completing the nomination form, it may be helpful to refer to the Social Care Wales Professional Codes of Practice for Social Care Workers. If relevant to fields such as nursing, you may also reference the NMC Standards of Conduct, Performance, and Ethics. Examples, where possible illustrate how the nominee:  -Engages and supports individuals receiving services, as well as care givers and others significant in their lives. - Acts as a role model, demonstrating person- and relationship-centered approaches, supporting colleagues, or displaying outstanding communication and leadership skills. - Upholds best practices by respecting diversity and promoting the independence, rights, and dignity of those they serve. -Collaborates effectively within and beyond the organization, supporting social care values and fostering public trust in the services provided.  These examples will help convey the nominee’s dedication to professional standards and ethical practices.
