Consent for Participation and Medical Treatment
I understand and acknowledge the following terms and conditions related to participation in activities organized by Play Tennis Academy:

Insurance Responsibility:
PTA does not provide student/player insurance. I am responsible for any medical expenses that may arise due to participation in this activity.

Acknowledgment of Risks:
I acknowledge that there are inherent risks involved in any athletic activity, including tennis.

Consent for Medical Treatment:
In consideration of myself/my child participating in this sport, I hereby consent to emergency medical treatment, hospitalization, or other necessary medical procedures by a physician and/or hospital in the event of injury or illness.

Waiver of Liability:
I waive any liability of Play Tennis Academy, its agents, or employees arising out of such medical treatment.

Safety Measures: PTA strives to provide a safe environment with approved and certified tennis coaches. While activities are designed to stimulate and challenge students/players, it is acknowledged that accidents may occur. Participation in supervised sport activities may present some risks. Although serious injuries are uncommon in supervised programs, it is impossible to eliminate all risks. Students/players are required to obey all safety rules, report any physical problems to the program or sport activity supervisor, and ensure the safe condition of their own equipment.
