Liability Waiver
I acknowledge that participation in activities with IamMore Foundation, a youth program initative of Warrior Development Institute, may involve minimal risks of injury, including but not limited to cuts, bumps, or other minor injuries. In the event of such occurrences, I agree that I will not hold IamMore Foundation, Ladies & Gents 2 Warriors DBA Warrior Development Institute, their staff, volunteers, or affiliates legally responsible, and I waive any right to pursue legal action for such minor injuries.*

As a participant in the IamMore Foundation and Warrior Mentality Youth Program, I agree to the following expectations:

I will treat myself and others with kindness, dignity, and respect.
I will listen when others are speaking and value their opinions.
I will respect program leaders, volunteers, and guest speakers.
Positive Attitude
I will come ready to participate with an open mind and a positive spirit.
I will encourage my peers and celebrate their successes.
I will avoid negative language, gossip, or bullying of any kind.
Responsibility
I will take responsibility for my actions and choices.
I will arrive on time and prepared for all program activities.
I will follow program rules and instructions for safety and success.
Personal Conduct
I will dress in a way that reflects self-respect and the values of the program.
I will use respectful language at all times (no profanity or inappropriate jokes).
I will not engage in disruptive, unsafe, or disrespectful behavior.
Commitment
I will give my best effort in all program activities.
I will be honest and trustworthy.
I will support my fellow participants so we can all grow together.

SIGNATURE OF AGREEMENT
I give permission for my daughter(s) to participate in the IamMore program and all related activities. The information I have provided is accurate to the best of my knowledge. I understand that reasonable precautions will be taken to ensure her safety throughout the program. I also acknowledge that participation may involve some minimal risks. I agree that both my child and I will adhere to all program guidelines, rules, and behavior expectations established by IamMore and Warrior Development Institute. I further understand that failure to follow program rules may affect my child’s continued participation in the program.*
