Acknowledgment of Risk
I understand that participation in Pilates exercises involves physical activity, which carries inherent risks including, but not limited to, muscle strain, joint injury, or other health complications. It is my responsibility to consult with a healthcare provider before participating if I have any medical conditions, injuries, or concerns.
Medical Clearance
I confirm that I am in good health and have consulted with my physician before participating in Pilates training, particularly if I have any pre-existing conditions, injuries, or other medical concerns.
Assumption of Risk
I voluntarily choose to participate in Pilates sessions led by Jessica Peterson, a trainee completing their 500 hour comprehensive Pilates certification. I fully assume all risks associated with my participation.
Release and Waiver of Liability
In consideration for being permitted to participate in Pilates sessions, I hereby release, waive, discharge, and covenant not to sue the instructor, their affiliates, agents, and the facility where sessions occur, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in Pilates sessions.
Agreement to Follow Instructions
I agree to follow the instructions provided by Jessica Peterson during training sessions and to notify the instructor immediately if I experience pain, discomfort, or any abnormal physical sensations.
Emergency Contact
In the event of an emergency during a Pilates session, I authorize the instructor to contact the person listed below. I understand it is my responsibility to keep this information current.
