HATHALETICS, LLC

to participate in the Hathaletics, LLC ("Hathaletics") yoga course ("Course" By signing this General Release and Waiver, I affirmatively state that I am the legal guardian of Minor and have the authority to sign the General Release and Waiver on the Minor's behalf, and further affirm that Minor is in good health and physical condition sufficient to participate in the Course. I further affirm, acknowledge and appreciate the following:

I. NOTICE: RISK OF INJURY

a. I understand that participation in the course includes the inherent risk involved with yoga and physical activity, such as serious or disabling injury.

b. I understand that, as a result of injury, medical treatment may be required and may be administered to the Minor. I understand that Hathaletics is not responsible for the medical treatment provided, and any risk of injury associated with medical treatment of the Minor.

c. I further understand that Minor is not required to participate in the Course in recognition of the possible and inherent risks associated with participation. I have informed Minor of such risks and have informed Minor that he/she may choose to participate in the Course or otherwise may limit his/her participation in recognition of his/her own physical abilities, condition and health.

d. By participating in the Course, I and Minor appreciate, understand, and acknowledge the inherent risks associated with participating in the Course.

II. PHOTOGRAPHY RELEASE: I understand that Hathaletics routinely takes videos andphotographs of Course participants. I hereby authorize and give my full consent toHathaletics to copyright and/or publish any and all photographs, videos, and/or film inwhich I appear while attending and/or participating in the Course. I further agree thatHathaletics may transfer, use or cause to be used, these photographs, videos, or films forinstructional, promotional or advertising purposes.

III. WAIVER AND RELEASE OF LIABILITY: To the fullest extent permitted by thelaws of the State of Connecticut, I hereby release Hathaletics, its members, officers,agents, employees and representatives from any liability whatsoever arising out ofMinor’s participation in the Course, and agree to indemnify and hold harmlessHathaletics from any claim made by any party arising out of Minor’s participation in the Course.

IV. GOVERNING LAW: This General Release and Waiver is governed by the laws of the State of Connecticut.

V. VENUE: Any claim arising out of Minor's participation in the Course, or any dispute arising out of this General Release and Waiver, or its applicability to any claim or dispute, must be filed with the Connecticut Superior Court, Judicial District of New London, or the United States District Court for the District of Connecticut.  

VI. SEVERABILITY: I understand and agree that, if any provision of this GeneralRelease and Waiver or the application thereof is held invalid or unenforceable, theinvalidity shall not affect other provisions or application of the General Release andWaiver which can be given effect without the invalid provisions or applications. Tothat end, the provisions of the General Release and Waiver are declared severable.
