I understand that Sunside Shih Tzu reserves the right to deny any application if the home is not deemed suitable or if there are no available puppies that match the applicant’s preferences.

I also acknowledge that, if approved, I will be required to sign a legally binding Puppy Sales Contract, agree to the spay/neuter requirement, and comply with all terms regarding proper care and rehoming as outlined therein.

Sunside Shih Tzu will make every reasonable effort to contact the potential buyer in a timely manner following receipt of the application
