THIS IS NOT A REGISTRATION FORM! This form is to be completed by a parent/guardian who is interested in enrolling their child/children in our reading tutoring program. Please allow up to 72 business hours to receive a phone call regarding your submission. After a successful call, you will be provided with the registration and payment link if you wish to proceed with enrollment.
