HATHALETICS Release and Waiver

(Print name) ("I", "me" or "Participant"), have elected to participate in the Hathaletics, LLC ("Hathaletics") yoga course ("Course" By signing this General Release and Waiver, I affirmatively state that I agree with the terms and conditions of General Release and Waiver, and further affirm that I am in good health and physical condition sufficient to participate in the Course. I further affirm, acknowledge and appreciate the following:

PHOTOGRAPHY RELEASE: I understand that Hathaletics routinely takes videos and photographs of Course participants. I hereby authorize and give my full consent to Hathaletics to copyright and/or publish any and all photographs, videos, and/or film in which I appear while attending and/or participating in the Course. I further agree that Hathaletics may transfer, use or cause to be used, these photographs, videos, or films for instructional, promotional or advertising purposes.

WAIVER AND RELEASE OF LIABILITY: To the fullest extent permitted by the laws of the State of Connecticut, I hereby release Hathaletics, its members, officers, agents, employees and representatives from any liability whatsoever arising out of my participation in the Course, and agree to indemnify and hold harmless Hathaletics from any claim made by any party arising out of my participation in the Course.

JURY CLAIM: I hereby knowingly, voluntarily and intentionally waive any right I may have to trial by jury in any action or proceeding of any kind or nature, in any court in which an action may be commenced, arising out of or in connection with this General Release and Waiver, my participation in the Course, or by reason of any other cause, claim or dispute whatsoever between the Participant and Hathaletics of any kind or nature.

SEVERABILITY: I understand and agree that, if any provision of this General Release and Waiver or the application thereof is held invalid or unenforceable, the invalidity shall not affect other provisions or application of the General Release and Waiver which can be given effect without the invalid provisions or applications. To that end, the provisions of the General Release and Waiver are declared severable.
