Checks: To pay by check, please make checks payable to Shoulder 360, Inc and mail to 5320 Ivafern Ln, La Canada, CA 91011. Checks must be recieved by April 13, 2026.
Cancellations: All conference cancellations must be made in writing by emailing to shoulder360course@gmail.com. Cancellations made by Monday, March 2, 2026 will receive a full refund minus a $30 processing fee. A 50% refund will be received if made between March 2 - April 3, 2026. Cancellations made after April 3, 2026 will not receive a refund.

- 360 Miami Vice Night: $110 (Friday, April 24, from 8:30–11:00 PM at the Temple House. Enjoy an open bar, desserts, music, and entertainment.)
