Client Agreement & Acknowledgment

By signing below, I acknowledge that I am choosing to receive a permanent jewelry service from Forever Fancy Permanent Jewelry. I understand that this is a cosmetic, non-invasive procedure involving the welding or fusing of fine metal chain ends together. This process does not pierce the skin and is designed for long-term wear. I agree to follow all aftercare instructions provided and understand that I may remove the jewelry at any time by cutting it safely.

Purpose of Consent

Permanent jewelry is a process that involves using a precision welding machine to join fine metal chains directly on your body. The process is painless, performed with safety in mind, and only by trained professionals. Although certification is not a legal requirement, Forever Fancy Permanent Jewelry has voluntarily completed professional training and certification to ensure the highest standards of safety and quality.

Acknowledgment of Risks

I understand and accept that:

- The welding process involves a minimal risk of heat exposure or minor burns.

- Jewelry may cause skin irritation, allergic reactions, or discomfort.

- Snagging or pulling may lead to skin irritation, injury, or jewelry damage.

- I am responsible for removing the jewelry safely if irritation occurs.

- Permanent jewelry may not be suitable for MRI scans, surgeries, or medical procedures.

Medical Disclosure

I do not have skin sensitivities, allergies, or medical conditions that could increase risk.

I have disclosed any known allergies or sensitivities to the jeweler.

I understand that this service is not recommended if I have skin conditions or wounds in the service area.

Release of Liability

I hereby release, discharge, and hold harmless Forever Fancy Permanent Jewelry, its owners, employees, and independent contractors from any and all liability, claims, demands, or causes of action that may arise from my participation in this service, including but not limited to injury, allergic reaction, or damage to personal property.

Consent to Service

I am voluntarily requesting this permanent jewelry service. I have had the procedure explained to me, understand the risks, and give my full consent to proceed. I acknowledge that I may remove the jewelry at any time by cutting it.

Allergic Reactions May Happen

Although rare, allergic reactions to jewelry metals may occur. Forever Fancy Permanent Jewelry uses nickel-free metals for all chains.

Rewelding Policy

If your jewelry comes off within 14 days of placement at the weld site only, please contact Forever

Fancy Permanent Jewelry for a free reweld. Claims must include a photo of the break and the entire chain. After 14 days, a reweld charge of $25 applies. All reweld requests and questions should be directed to alicia@foreverfancypermanentjewelry.com or through our website at foreverfancypermanentjewelry.com.

Refund Policy

Permanent jewelry is custom-fit to each individual. Once your chain is cut to size and welded, all sales are final. A $25 reweld fee applies for jewelry needing service after 14 days.

Warranty Clause

Forever Fancy Permanent Jewelry provides a 14-day warranty covering defects at the weld site only.

This warranty does not cover lost, stretched, or snagged chains. Claims must be submitted with proof of purchase and a photo of the jewelry within 14 days of service.

Acceptance

I have read and agree to the terms of this consent form. I have had the opportunity to ask questions and have received satisfactory answers. I understand the risks and potential side effects associated with the permanent jewelry process. I am over the age of 18 and consent to this service. If under 18, a parent or guardian must also sign below.
