    You must complete this form if you wish to hand in a dog to our rescue. Please do not call the centre if you need to rehome your dog. This takes valuable time away from caring for our rescue dogs and we will only refer you back to this page unless you are having difficulty completing the form. Please also note we only take in German Shepherds, at this time we cannot take in cross breeds. We may be able to recommend an alternative rescue if you have a cross breed – contact us via the website chat.

    The detailed application form needs to be completed as accurately and as honestly as possible to ensure we can offer the best care for the dogs we take in. If there are inaccuracies we may not be able to look after and rehome the dog suitably which could be detrimental to their welfare and that of other dogs around them. If you have no history on some of the behaviours required in the form, please just complete 'unknown'.

    Please Note: Due to our limited funds, we ask for a donation from private homes when a dog is handed in. This is a contribution towards any veterinary care required (health checks, vaccinations, flea, worming, neutering etc.) food costs and general upkeep and care of the dog during their stay with us.

    Every dog that comes into our care costs the rescue on average a minimum of £3,500 per year.  This covers food and everyday welfare, this does not take into account individual vet visits and treatments that the dog may require, some of which can far out weigh this average cost meaning that the rescue could be out of pocket £5,000 - £10,000 per condition.

    On completion of the form you will receive an acknowledgement of your form being received and your dog will then be added to our waiting list. Our rehoming team will then work out the priority for your application and assign your position on the waiting list accordingly, as part of this we may request you bring the dog in for an initial assessment to assess where we could accommodate them. We cannot usually take the dog immediately and so you would have to take them home again. If we can take your dog we will then arrange a time for you to bring them in. 

    Please note before we take in any dog we will require an up to date vet report on the dogs general health which will require an up to date vaccine.

    Although we try to move dogs into new homes as quickly as we can in a responsible way, depending on their issues, this sometimes can take months if not years, especially with the dogs that are in need of major rehab before being allowed up for adoption.

    Therefore we are grateful for every donation that comes into the charity pot, in the case of incoming dogs we ask that the owners or family handing them over contribute towards these future costs by making a minimum donation of £150 when the dog is handed over.

    This initial money will stay with your dog for initial vet appointments etc

    We truly understand how heartbreaking this situation is. However, due to your dog’s history, medical condition, and age, finding an alternative placement is extremely difficult and could put them at serious risk of being put to sleep by others who do not know them.

    Please remember that rehoming a dog with known bite issues can also lead to serious legal consequences if they were to bite again.

    As painful as it is, sometimes the kindest and most loving choice is to let them go peacefully—at home, surrounded by the people they trust and love most. Being there for your dog in their final moments ensures they feel safe and cared for, rather than confused or frightened in unfamiliar surroundings.

    Thank you again for caring enough to seek help. Making this decision takes immense love and courage, and we hope you find comfort in knowing that you are doing what’s truly best for your dog.

I understand that full disclosure of such information is essential to ensure the safety and welfare of my dog and any potential new owners. I acknowledge that the information provided will not negatively affect my dog’s consideration for rescue or rehoming.

    I hereby grant LUOSKO German Shepherd Dog Rescue permission to use the information contained within this form (excluding any personal contact details) for the purpose of promoting the rehoming of my dog via the organisation’s website, social media channels, and other promotional outlets as deemed appropriate by the rescue.

    I agree to refrain from commenting publicly or privately on any posts, listings, or online discussions relating to the rehoming of my dog. I fully understand that any attempt to interfere with, influence, or undermine the rehoming process may result in LUOSKO German Shepherd Dog Rescue immediately withdrawing all promotional material and ceasing any further assistance with rehoming my dog.

    I acknowledge that LUOSKO German Shepherd Dog Rescue may promote my dog for rehoming prior to a full intake and that I may be required to present my dog for assessment before the rescue formally accepts them into its care.

    I further agree not to advertise, rehome, or attempt to rehome my dog through any other organisation, platform, or private arrangement without first notifying LUOSKO German Shepherd Dog Rescue in writing. I understand that failure to provide such notification will result in LUOSKO ceasing all efforts and promotional activities related to the rehoming of my dog.

    As of today’s date, I formally authorise LUOSKO German Shepherd Dog Rescue to act as the sole organisation responsible for assisting in the rehoming of my dog. I understand that I may revoke this authorisation at any time by submitting written notice to rehoming.team@gsdrescue.uk, providing a minimum of seven (7) days’ notice prior to the intended revocation date.
