1. Purpose of Service
The Model acknowledges and understands that the procedure is being provided as part of the Artist’s professional training, portfolio building, and business development.
This service is offered at a discounted or complimentary rate as a model session and may be utilized to refine skills, test new techniques or products, and generate promotional materials for business growth.
The Model understands that results may vary as this procedure is performed in a model capacity and that additional sessions may be required to achieve the desired outcome.
2. Recording and Content Release
The Model acknowledges that photo, video, and/or audio recording will take place during the session. These recordings are an essential component of the Artist’s documentation, education, and marketing process.
By signing this form, the Model grants the Artist and Business irrevocable permission to record, photograph, edit, and use any visual or audio material captured during the session for purposes including, but not limited to, portfolio display, social media, advertising, education, and promotional materials, in perpetuity and without compensation.
The Model waives all rights to inspect or approve the final use of these materials and understands that ownership of all media content remains with the Artist and Business.
3. Health and Medical Disclosure
The Model affirms that all personal, medical, and health information provided to the Artist is accurate and complete to the best of their knowledge. The Model understands that failure to disclose any medical conditions, medications, allergies, or recent health events may increase the risk of adverse reactions.
The Model acknowledges that the Artist is not a medical professional and that permanent makeup procedures carry inherent risks, including but not limited to:
Allergic reaction to pigment or topical agents
Skin irritation, swelling, or infection
Uneven or undesired pigment retention
Possible need for touch-ups or corrections
The Model agrees to notify the Artist of any medical concerns that may arise before or after the procedure and to seek medical attention if necessary.
4. Aftercare and Results
The Model confirms receipt of written and/or verbal aftercare instructions and understands that proper aftercare is essential for healing and pigment retention.
The Artist makes no guarantee as to the precise outcome or longevity of the procedure, as results vary by individual skin type, lifestyle, and healing response.
5. Liability Waiver and Consent
The Model voluntarily consents to receive the permanent makeup procedure with full understanding of the nature and potential risks involved.
By signing this document, the Model releases, waives, and forever discharges the Artist, the Business, and any affiliates, employees, or representatives from all liability, claims, or damages that may arise from participation in this procedure, including the use of photographic or video materials.
The Model confirms that they are:
At least 18 years of age
Not pregnant or nursing
Not under the influence of drugs or alcohol
Mentally competent to provide informed consent
The Model acknowledges that they have had the opportunity to ask questions, that all questions have been answered to their satisfaction, and that they understand and agree to the terms of this release.
6. Acknowledgment
By signing below, the Model affirms that they have read, understood, and voluntarily agree to all terms and conditions outlined in this release form.
