Group Visit

      Each group must include at least 2 people (new or returning). 

    Fees are based on number of patients scheduled. Whomever schedules the appointment is responsible for the fees of participants who are absent. 

    Returning patients: Each $40
    New patients: $60 

    All new patient must complete the New Patient Intake Form which will be included in the confirmation email.
