Welcome to the Sweetest Photo Booth Ever!
Thanks for choosing Forget Me Not Cookie Co. to make your event extra sweet! This form will guide you through sharing your event details, cookie preferences, and any optional add-ons to bring a sweet, interactive photo booth to your next event! A few important notes before you begin:
- Deposit: A deposit equal to the base cost of your package (printing time + mileage + upgrades) secures your date. It’s fully refundable if canceled at least 2 weeks before your event, and 50% refundable if canceled 7-13 days before. Don’t worry — rescheduling is always an option!
- Final RSVPs: Please submit your final serving count at least 1 week prior to your event, which is when we will invoice for the cookies themselves.
- Participant Payment: We’ll invoice you for the final cookie count a week before the event. Payment is due within 2 days.
- Special Dietary Requests: We welcome them at no extra charge (keto/sugar-free treats are the only exception).

Once your form is submitted, we will review your request, confirm availability, and clarify any details before we send you an invoice that must be paid within 2 business days to secure your date.
