Achieve Academy Enrollment Contract for the 2026/2027 School Year

This contract is made between Achieve Academy (referred to as “The School”), 1602 Elberta Rd., Warner Robins, GA 31093 and the parents/guardians of the student(s) named below (referred to as “The Parents”).

All persons signing this Contract are jointly and severally liable for the tuition and fees set forth herein. Parent’s signature and/or initials on this Contract evidence Parent’s understanding and agreement to the terms of this Contract, as follows:

1. Enrollment: Student will be enrolled for all of the 2026 - 2027 academic year which runs from August 1st, 2026 to July 31st, 2027. Tuition for the 2026 -2027 school year is $7,560. Parents are obligated to make twelve monthly payments to Achieve Academy of $630 per month from August 1st, 2026 to July 31st, 2027 or ten monthly payments of $756 from August 1st, 2026 to May 31st, 2027. Promise Scholarship recipients are obligated to pay the difference between the scholarship amount and tuition plus fees. The Contract is valid only for the academic year stated and does not entitle Student to any future enrollment. Initial below

2. Registration Fee: Parent understands that for the School to consider Student’s application and to reserve a place for Student for the academic year stated above, Parent must submit the original executed Contract. A non-refundable registration fee of $85 is payable yearly for each student at the time of enrollment or re-enrollment. Initial below

3. Tuition: Tuition amounts must be paid in full, monthly; an outstanding balance cannot be applied to the following month. In cases where the tuition is not paid by the end of the month in which it is due, student shall be unable to attend or participate in school activities. Initial below

4. Tuition Obligation: Parent understands that enrolling a student reserves a place for the entire school year (or contract period). The School’s operating costs remain the same even if a student withdraws, so early withdrawal causes a financial loss that cannot be easily calculated. For this reason, once this Contract is submitted, the Parent is responsible for the full year’s tuition and fees, even if the student is withdrawn, absent, or dismissed, unless the School declines the student’s enrollment. If the student leaves the School for any reason, including but not limited to relocation, health issues, withdrawal, or expulsion, there will be no refund or reduction of tuition or fees. Any remaining balance may become immediately due. Initial below.

5. Force majeure: Neither party will be held responsible for delays or failure to perform obligations caused by events beyond their reasonable control. These events may include, but are not limited to, natural disasters, acts of war or terrorism, shortages in supply, power failures, computer or data system malfunctions, labor disputes, or civil unrest. If such an event occurs, both parties agree to make a good faith effort to fulfill their responsibilities under this Contract. In the event of a recurrence of COVID-19 or another virus, disease, pandemic, or unexpected event, the School reserves the right to continue services through alternative methods, such as distance learning or home study packets. 6. Summer School. Parent understands that summer school is mandatory for student attendance, unless the option of ten monthly payments has been preselected and adhered to throughout the school year. Initial below

7. Book & Supply Fee: Parents agree to pay a one-time annual book fee and school supplies fee for each enrolled student. The book fee is $240 for students in first grade or below and $340 for students in second grade and above. The school supplies fee is $65 per student. Both fees must be paid in full by June 16, 2026. 8. Additional Fees and Incidentals: Parents agree to pay the School for incidental fees, which may include interest, late fees, overdue library fees, unreturned textbooks, before and after school care, and field trips. Initial below

9. School Rules: Student’s enrollment at the School is subject to the general statements, rules, regulations, conditions, and financial terms contained in the School’s Parent and Student Handbook and other published documents, which may be amended from time to time. Parent acknowledges that Parent and Student must abide by such School rules and guidelines. Initial below

10. Parent Support & Participation: Student enrollment is contingent upon the Parent’s support of the School’s philosophy, methods, objectives, and policies, including its moral, academic, behavioral, dress, conduct, and disciplinary standards. Parents agree to monitor and encourage their child’s education, maintain regular communication with teachers, attend all mandatory meetings called by the School, and support the School’s overall program through time, attendance at parent meetings, and participation in various School activities. 11. Grounds for Suspension or Termination: The School has the right to suspend or terminate the attendance of any student for reasons outlined in the Parent and Student Handbook or other official documents, for conduct that the School Administration determines to be detrimental to the School community, to the student, or to other students. This right also extends to cases in which the Parent fails to meet financial obligations for the Student’s attendance or fails to uphold the responsibilities of partnership with the School, such as maintaining communication, cooperation, and appropriate support for the Student’s education and the School’s standards. 12. Payment and Late Fees: Parent understands and agrees that a late charge of $50 will be added to any payment not received within four (4) days after the due date. If payment is not received within eight (8) days after the due date, an additional $50 late charge will be applied, for a total of $100 in late fees. If any balance remains unpaid thirty (30) days past the due date, the School reserves the right to suspend or discontinue services until the account is brought current. Parent further agrees to pay all costs of collection, including collection agency fees, attorney’s fees, court costs, and interest of 1.5% per month. If an account becomes ninety (90) days past due, the School reserves the right to accelerate the total remaining balance of tuition and fees under this Enrollment Contract. Accounts in arrears may be referred to the School’s collection agency or legal counsel for satisfaction. Initial below.

13. Default of Payment: All accounts must be current before records and transcripts can be released or transferred to other schools. Student will not be allowed to continue to attend classes or participate in other School activities unless tuition and fees are paid by stated deadlines (or until Parent makes other written arrangements acceptable to the School). 14. Photos and Images: The Parent agrees to allow the Student’s name, photograph, voice, image, and information to be used by the School for use in the School’s publications, promotion materials, social networks, and website, without compensation and without prior notice. Parent also allows Student to be interviewed by the media on campus or at school-related events. Parent releases and holds the School harmless from any liability stemming from the use of the Student’s name, photograph, voice, image, or information. Initial below

15. School Directory: Parent authorizes the School to place family information, including name(s), home address(es), email address(es), and telephone numbers of Parent, Student, and other children in attendance at the School, in a directory of students to be available to administration, teachers and substitutes. Parent acknowledges that this directory and the information therein is not to be used for commercial use and is not to be distributed to any other person or agency. Initial below

16. School/Family Cooperation: A positive and cooperative relationship between the School and each Family Member (defined as Parent, Student, or any person associated with the Student) is essential to fulfilling the School’s educational purpose. If any Family Member engages in behavior, communication, or interactions, on or off campus, that are disruptive, intimidating, aggressive, or show a loss of confidence in or disagreement with the School’s policies, instruction, or discipline methods, or that otherwise interfere with the School’s safety, responsibilities, or educational mission, the School reserves the right to dismiss the Family or Family Member from the School community. The School may also restrict a Family Member’s participation or presence on campus for any reason it deems appropriate. Such determinations are made at the School’s sole discretion. In cases of dismissal, no tuition or fees will be refunded, and any unpaid balance will remain due in accordance with this Contract. The School also reserves the right to withdraw an offer of enrollment or re-enrollment at any time and to void an executed Enrollment Contract. Initial below.

17. Medical Care & Authorization: The Parent authorizes the School to provide medical care for the Student as needed, including the administration of prescribed medications (such as allergy medications or EpiPens), minor first aid, or other emergency treatment deemed appropriate by School staff. If, in the opinion of a licensed physician, the Student requires medical or surgical care that needs parental authorization, the Parent hereby authorizes the School to give such consent on the Parent’s behalf so the Student may receive necessary treatment as soon as reasonably possible.The Parent releases and holds the School harmless from any liability that may result from the provision or authorization of such care and agrees to reimburse the School for any medical expenses incurred on the Student’s behalf. This authorization also extends to referrals for Student counseling services as deemed necessary by the School. Initial below.

19. Transfer Records & Enrollment Conditions: If the Student is transferring from another school, it is the Parent’s responsibility to ensure that the previous school promptly provides the School with an official transcript. Enrollment is also contingent upon the Student’s satisfactory completion of the current school year in good academic and behavioral standing. If, after reviewing the Student’s records, the School determines in its sole discretion that these conditions have not been met, the School reserves the right to unilaterally cancel this Contract. 20. Release of Student Records: Parent consents and holds the School harmless for the release of Student’s records and information upon request by an educational institution or law enforcement agency. Parent also releases and holds the School harmless from any liability stemming from the use, disclosure, or release of Student’s records or information. Initial below

21. Promotional Materials & Accuracy: The School strives to ensure that all written materials, such as brochures, handbooks, and advertisements, are accurate. However, details such as class sizes, student-to-teacher ratios, accreditation, and staff qualifications may change as programs and personnel evolve. Parents should verify any important information with the Principal before enrolling, as only the Principal or their designee is authorized to make commitments or exceptions to the School’s regular curriculum. 22. Fundraising Efforts: Parents agree to participate in all fundraising efforts on behalf of Achieve Academy and Achieve Foundation.

23. Reimbursement for Domestic Legal Issues: The Parent understands that the School’s role is to provide education and not to become involved in domestic legal disputes between Parents. If the School or its employees are required to participate in such matters, including but not limited to custody, divorce, or educational placement disputes, the Parent agrees to promptly reimburse the School for all related expenses. These may include reasonable attorney’s fees, staff time, document preparation, travel, and other associated costs. Payment for such expenses is due within thirty (30) days of billing. Any disagreements between Parents regarding responsibility for these costs must be resolved between them. Failure to pay promptly may result in dismissal of the family from the School. 24. Parent’s Commitment to Truthfulness: The School relies on the accuracy and completeness of all information provided by the Parent during the admissions and enrollment process. If the School later determines, in its sole discretion, that any information was false, misleading, or omitted in a way that the School considers material, the School reserves the right to terminate this Contract. In such cases, no tuition or fees will be refunded, and any unpaid balance will remain due under this Contract. Initial below.

25. Governing Law, Understanding of Terms, and Entire Agreement: This Contract shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to principles of conflicts of law. By signing below, the Parent acknowledges that they have read and understand all terms of this Contract, including the obligation to pay the full year’s tuition even if the Student is withdrawn or dismissed, and all other obligations set forth herein. The Parent is encouraged to seek clarification from the Principal or legal counsel if any part of this Contract is unclear. This Contract represents the entire agreement between the parties regarding the Student’s enrollment and supersedes all prior or contemporaneous oral or written understandings, representations, or warranties not contained herein.
