Be sure to print a copy of your form before you submit to retain for your reference since you will not be able to return to the form once it is submitted. Please preview your print & adjust the % size to ensure you will not "cut off" any part of the form before printing (generally, reducing the size down to 80% will work).
If you need to change your entry, please email Deb
If your entry is successful, you will see a "Thank you" screen after you click on the Submit button and complete your online registration. You will also receive a confirmation email within a few hours. If you do not see the "Thank you" screen and receive the confirmation email, please assume that your entry was not received and email Deb for assistance.
