By submitting this form, the undersigned (“Participant”) agrees to the following terms and conditions: I understand that participation in Hawthorne Eats: Restaurant Week is voluntary and that my business assumes all risks associated with participation, including but not limited to property damage, loss of revenue, or any injury or illness to myself, my employees, or customers arising from our participation. I hereby release, indemnify, and hold harmless the City of Hawthorne, its officers, employees, agents, volunteers, sponsors, and event partners from any and all claims, damages, losses, liabilities, costs, or expenses (including attorney’s fees) that may arise out of or relate to our participation in this event. I acknowledge that my business is solely responsible for ensuring compliance with all applicable health and safety standards, city and county regulations, and current Department of Public Health requirements. I also understand that inclusion in the event’s marketing materials or website does not imply endorsement or inspection by the City of Hawthorne. I certify that I am an authorized representative of the participating business and that I have read, understood, and agree to the terms of this waiver and indemnification agreement.
