Burhan Blazers Basketball Academy
Welcome to Burhan Blazer Basketball Academy! Our goal is to introduce young athletes to basketball in a faith-based environment with competition, fun and education. We will be doing some fundamentals and skills training each week both to determine level of play and to build on concepts as the academy continues. Kids will get a chance to practice and then put it to the test in a real game under ref and whistle. This is especially valuable as early on all players don’t grasp the concepts at the same rate. To be able to have the game broken down, demonstrated and then go out and compete, is usually received and processed at a higher level. Our coaches and refs will promote sportsmanship, effort and teamwork as we have fun playing basketball and making new friends. Note to Parents: Advanced players may be challenged or asked to play in different divisions to match their skill sets.

A PARENT MUST SIGN AND DATE HERE AFTER READING: Please read this waiver carefully. Signing your name means you accept the terms and conditions of the waiver. Burhan Academy Sports Camp Waiver and Release of Liability*Acknowledgment and Assumption of Risk *I, the undersigned parent/guardian of the above-named participant, acknowledge that participation in sports camps and related activities at Burhan Academy involves inherent risks, including but not limited to physical injury, illness, or other unforeseen hazards. I understand that while Burhan Academy strives to provide a safe environment, it cannot guarantee the elimination of risks. By signing this waiver, I voluntarily assume all risks associated with my child’s participation in the sports camp, including but not limited to: Injuries resulting from physical activities, such as running, jumping, or playing sports. Injuries caused by equipment or facilities. Exposure to communicable diseases, including but not limited to COVID-19.Waiver and Release of Liability In consideration of my child’s participation in this sports camp for primary and secondary education students, I hereby release, discharge, and hold harmless Burhan Academy, its staff, volunteers, representatives, and affiliates from any and all claims, liabilities, or damages arising out of or related to my child’s participation, except where caused by gross negligence or willful misconduct. Medical Treatment Authorization In the event of an emergency, I authorize the sports camp and its staff to secure appropriate medical treatment for my child, including transportation to a medical facility. I agree to be responsible for any medical expenses incurred. *Code of Conduct Agreement *I acknowledge that my child is expected to adhere to the rules and guidelines established by Burhan Academy. Failure to comply with these rules may result in removal from the camp without refund. Acknowledgment of Understanding I have read and fully understand this waiver and release of liability. By signing below, I confirm that I am the legal parent/guardian of the participant and that I voluntarily agree to the terms outlined in this document.
