When possible, please attach supporting documents such as photographs or diagrams. On the back of all supporting documents note the Unit and Lot number of the property the Report of Violation is about. Please be aware these documents will not be returned. Be as brief as possible with only one Violation listed per form.
In accordance with Arizona law, 33-1803, included in the Notice of Violation mailed to the alleged violating party, the HOA is required to release the first and last name of the reporting party. Only written Report of Violations will be accepted by HOA #2 Architectural & Landscaping Committee, anonymous or oral Report of Violations will not be taken under consideration.
