    Thank you for letting us know you need to make a change for the 2026 Winter Session! ALL schedule and withdrawal requests must be made using this form by December 2 to be considered. Students without valid change request forms by December 2 will be confirmed for Winter Enrollment and tuition charged to the stored payment method. Please enter the same email address for the primary adult contact that is used for your registration in Pike13. If you are making changes for multiple students, please complete a separate form for each student.
