1. I authorize Plot Twist K9 Services LLC to perform agreed grooming services on my dog, including bathing, brushing, haircut, nail trimming, ear cleaning, and related care.
2. Health & Behavior
I confirm my dog is healthy, up-to-date on rabies vaccination, and free of parasites. I have disclosed any health or behavioral concerns.
3. Matted Coats & Special Handling
I understand removing mats may cause skin irritation or nicks. Plot Twist K9 Services will use professional judgment to ensure my dog's comfort and safety.
4. Emergency Care Authorization
If a medical emergency occurs, I authorize Plot Twist K9 Services to obtain veterinary care for my pet up to $1,000. I am responsible for all related costs.
I grant Plot Twist K9 Services permission to use photos/videos of my dog for business and social media purposes.
I release Plot Twist K9 Services, its owners, and employees from any liability for injury, illness, or death of my pet, except in cases of gross negligence or willful misconduct.
Payment is due at pickup. I understand that missed or late cancellations (less than 24 hours' notice) may result in a fee.
