High Desert Second Chance – Release and Waiver of Liability
By submitting this form, I acknowledge and agree to the following terms and conditions regarding participation in any event, class, workshop, or program offered at or through High Desert Second Chance (HDSC), whether hosted directly by HDSC or by a partner agency or third-party organization.
Voluntary Participation
I understand that participation in any HDSC-affiliated program, event, or service is completely voluntary. I choose to participate at my own discretion.
Independent Partners and Third Parties
I acknowledge that some programs, services, or activities offered at HDSC are conducted by partner agencies or third-party organizations operating independently from HDSC. HDSC does not oversee or control these entities, their employees, volunteers, or representatives.
Assumption of Risk
I understand that participation in any activity involves potential risks, including but not limited to personal injury, illness, property damage, or other unforeseen circumstances. I voluntarily assume all risks associated with participation.
Release of Liability
I hereby release, waive, and discharge High Desert Second Chance, its officers, directors, employees, volunteers, agents, and representatives from any and all claims, demands, losses, liabilities, damages, or expenses (including attorney’s fees) arising from or related to my participation in any HDSC event or any program provided by a partner agency or third party.
Medical Treatment
I understand that HDSC does not provide medical care or insurance coverage for participants. In the event of illness or injury, I am responsible for any medical treatment or expenses incurred.
Photo/Media Release
I grant permission for HDSC and its partners to use photos, videos, or recordings taken during participation for promotional or educational purposes, without compensation.
Acknowledgment
By submitting this form, I confirm that I have read and understand this Release and Waiver of Liability, and agree to its terms freely and voluntarily. I understand that this waiver is binding to the fullest extent permitted by law.
