Project Starfish Admission 2026 Application Form

This form will take approximately 30 - 45 minutes to complete. All information provided are private and confidential. All sections with asterisk (*) are required fields.
YOU WILL NEED THE FOLLOWING INFORMATION TO COMPLETE THE FORM:

Referrers (school welfare officers/probation officers/social workers etc.) are advised to assist their referred youth with this form.
1. Youths' Personal Particulars
2. Youths' PSLE and most recent school grades
3. Parents'/Guardian's Personal Particulars
4. Youth's Household Income
5. Referrer's Particulars & Statement (applicable to those with referral)
6. Important dates (for those applicable) e.g. school withdrawal date, end
probation date etc.
7. A Gmail account (please ensure that you can log into your account)

To be eligible for the programme, the youth must be withdrawn from school or be in the process of withdrawing from school. If the youth is not withdrawn from school yet at the time of application, they must be officially withdrawn from school by mid February 2026 in order to proceed with the programme.

IMPORTANT NOTES:
• You are not able to amend the form after submission.
• If you need to amend significantly after submission, we advise you to re-do your
submission.
• Alternatively, for small changes, please inform us during the interview day itself.

This form must be completed before a briefing and an interview is granted.Please fill up this form by 14 November 2025. Any forms submitted after 14 November, will be considered only if we have vacancies available.Thank you.

Do note that incomplete submissions will be forfeited. If you wish to withdraw your application, kindly contact us at hello@starfishsingapore.org.

Section J: Admission Policy
Admission of students to Project Starfish is based on their academic results, interview evaluation and placement tests conducted by Starfish Singapore. Starfish Singapore reserves the right to reject any application, terminate any of its courses at its discretion and dismiss students who fail to abide by the Starfish Singapore's rules and regulations.

Section K: Declaration
By submitting this application,

(a) I consent to allow Starfish Singapore to collect, use, disclose and/or process my personal data in order to process, administer, facilitate, maintain and/or manage my relationship with Starfish Singapore as a programme participant or beneficiary, including communicating on Starfish Singapore’s activities (through phone calls, text, emails, or postal mail), programmes and services; carrying out research, analysis and development activities for Starfish Singapore’s purposes; for verification purposes and making disclosures required by law or a competent authority.

(b) I also consent to allow Starfish Singapore to use photos and videos that includes me, as well as, audios containing my voice and my personal statements taken during the programme for publicity and marketing purposes.

(c) I declare that all information and any additional documents given in this application are true and accurate to the best of my knowledge at the time of completion.

(d) I accept that if any of the information given by me in this application is in any way false or incorrect, my application may be rejected and any offer of acceptance may be withdrawn or my enrolment into the programme may be terminated.

(e) I understand that the approval of my application is subjected to the consideration of Starfish Singapore and if accepted into the programme, I agree to abide by the programme rules and regulations and fulfil the responsibilities of a student of Project Starfish to the best of my ability.
