Executive Director Nomination Form

Nominations are to be forwarded in writing, via this form or using the paper form, to the IHSA Secretary. Nominations close Friday 7th November 2025.

All nominations must be received no later than 35 days prior to the AGM (scheduled for 12 December 2025). 

Note: All signatories are required to be financial members of Ice Hockey SA – (SAIHA Inc.)

Please ensure you have discussed this nomination with the person and have their permission to submit on their behalf. They will be required to accept this nomination prior to the deadline. 

How does this form work?

Section One is to be completed by the person/s submitting the nomination. Enter your details along with those of the nominee and select SUBMIT.

The nominee will receive an email advising them of the nomination.The nominee will be asked to edit the submission and accept the nomination

Section Two is to be completed by the Nominee to accept the nomination. Once they accept and resubmit the nomination, the process is finalised. 

If you are completing the form as the nominee, you must enter details of two parties who support your nomination and accept the nimination.
