This Agreement allows your swimmer to participate in a 1-week Practice Trial with Arizona Tritons for evaluation purposes only. By completing this form, you—the parent/guardian (or adult participant) acknowledges the risks associated with participation in activities conducted by Arizona Tritons and Triton Elite Coaching Services LLC at Queen Creek Aquatic Center and understands that participation does not guarantee team membership.

1) ASSUMPTION OF RISK
Iclick to fill (parent/guardian) of click to fill (athlete) understand and acknowledge that participation in swimming, dryland training, travel, and related activities with the Arizona Tritons Swim Team ("AZ Tritons"), operated in association with Triton Elite Coaching Services LLC ("TECS"), and conducted at the Queen Creek Recreation and Aquatic Center ("QCRAC"), owned and operated by the Town of Queen Creek, Arizona, as well as in events sanctioned by USA Swimming, Arizona Swimming, and other meet host clubs and facilities, involves inherent and potential risks of serious injury, illness, or property damage.

These risks include, but are not limited to: drowning, slips and falls, overexertion, collisions, heat-related illness, communicable disease exposure (including COVID-19), and hazards associated with training, competition, and volunteer service.

I knowingly and voluntarily assume all such risks, whether caused by my own actions, the actions of others, environmental conditions, or the ordinary negligence of AZ Tritons, TECS, QCRAC, the Town of Queen Creek, USA Swimming, Arizona Swimming, the AZ Tritons Board of Directors, parent volunteers, meet hosts, or any affiliated individuals or organizations.
2. RELEASE OF LIABILITY
In consideration for being permitted to participate in or volunteer for AZ Tritons programs, I, for myself and/or my minor child, and on behalf of my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless:
- Arizona Tritons Swim Team,
- Triton Elite Coaching Services,
- The AZ Tritons Board of Directors,
- Queen Creek Recreation and Aquatic Center,
- Town of Queen Creek (Arizona)
- USA Swimming,
- Arizona Swimming,
- All meet host clubs, facilities, and organizations, and
- All parent and team volunteers,
together with their respective officers, directors, owners, coaches, employees, agents, contractors, and sponsors (collectively, the “Released Parties”), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, disability, death, or property damage occurring during or resulting from participation in AZ Tritons, TECS, or USA Swimming–sanctioned activities.

This release applies to all acts or omissions of the Released Parties, including ordinary negligence, but does not extend to gross negligence, reckless conduct, or intentional misconduct.
3. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, damages, liabilities, losses, or expenses (including attorney’s fees) arising from or related to my or my child’s participation in AZ Tritons, TECS, or USA Swimming–related activities, or from my actions as a parent on deck.
4. MEDICAL CONSENT AND EMERGENCY AUTHORIZATION
I certify that I and/or my child are physically fit and medically cleared to participate in swim training, dryland, and competition. In the event of injury or emergency, I authorize AZ Tritons, TECS, facility personnel, or medical professionals to obtain or provide emergency care as deemed necessary, including transport to a medical facility. I understand that I am financially responsible for all medical expenses incurred.
5. FACILITY USE AND EQUIPMENT
I agree to comply with all safety rules, conduct policies, and facility regulations established by AZ Tritons, TECS, QCRAC, the Town of Queen Creek, USA Swimming, and any meet host facilities. I understand that any property damage caused by my or my child’s negligent or intentional actions may result in financial responsibility.
6. MEDIA RELEASE
I grant permission for AZ Tritons, TECS, USA Swimming, Arizona Swimming, and their affiliates to use photographs, videos, or likenesses of myself and/or my child for promotional, educational, and social media purposes without compensation.
I decline media use of my child's image and understand that they may be separated from the group during photo related activities.
7. GOVERNING LAW
This agreement shall be governed by and construed under the laws of the State of Arizona. Any disputes shall be resolved exclusively in the courts of Maricopa County, Arizona. If any provision of this waiver is found invalid, the remaining provisions shall remain in full force and effect.
8. ACKNOWLEDGEMENT OF UNDERSTANDING
I have carefully read and fully understand this Liability & Medical Waiver, and I acknowledge that by signing it, I am voluntarily giving up substantial legal rights, including the right to sue the Released Parties. I sign this document freely and voluntarily, intending it to be a legally binding agreement enforceable under Arizona law.
