Waivers, Authorizations and Release of Liability

Acknowledgement of Risk and Participation
I understand that participation in Modern Motion Day Camp involves physical activity and outdoor experiences, including art, dance, sports, tumbling, and nature exploration. I recognize that these activities carry certain inherent risks, including the possibility of injury or illness. I voluntarily allow my child to participate in all camp activities and agree to assume all associated risks.

Release of Liability
In consideration of my child’s participation, I release and hold harmless Modern Motion, its owners, directors, staff, and volunteers from any and all claims, demands, or causes of action arising from injury, illness, or property damage occurring during camp activities, except in cases of gross negligence or intentional misconduct.

Medical Authorization
In the event of illness or injury, I authorize camp staff to administer basic first aid and, if necessary, obtain emergency medical treatment for my child. I accept responsibility for any costs related to such treatment.

Media Release
I give permission for photos, video, and quotes of my child to be used by Modern Motion for promotional or educational purposes in print, online, and on social media. (If you do not consent, please notify the office in writing.)

Code of Conduct and Communication
I agree to support the camp’s Code of Conduct and understand that disrespectful, unsafe, or disruptive behavior may result in dismissal without refund. I understand that camp updates and notifications will be sent via email and that it is my responsibility to keep my contact information current.

Acknowledgement and Acceptance
By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver and release. I certify that I am the parent or legal guardian of the participant and am authorized to sign on their behalf.
