PROGRAM RELEASE AND WAIVER OF LIABILITY
I understand that the Ride-A-Long and Job Shadow/Intern Programs are sponsored by the Village of Algonquin Police Department. I understand that the program I am participating in is being offered as a courtesy service by the Village of Algonquin Police Department. I further understand that the Village of Algonquin and any of its Officers, employees or agents who may provide such a service accept no responsibility for any loss, damage or injury which may result from the performance of this service.
In signing this authorization, I hereby forever release the Village of Algonquin, its Officers, employees and agents from any and all liability, damages, suits, claims and demands for damages at law or in equity including, but not limited to, personal injury, death or property damage, which I now have or may have hereafter on account of or in any way related to the performance of this service. I accept the service performed under the conditions stated above and assume full responsibility for any consequences, actual or implied. In signing this authorization, I also agree to indemnify and hold harmless the Village, its Officers, employees and agents from any and all liability, damages, suits, claims and demands for damages at law or in equity which someone other than myself may have or may have hereafter on account of or in any way related to the performance of this service.
