This form is an appointment request, not a guaranteed booking.
• Once I review your idea, I’ll contact you with pricing, available dates, and any questions.
• My minimum is $150 for simple tattoos less than 1.5 inch.
• A non-refundable deposit is required to secure your appointment.
• The deposit goes toward the final price of your tattoo.
• If you need to reschedule, please let me know at least 48 hours in advance so your deposit can be transferred to a new date one time.
• Same-day cancellations and no-shows may result in losing your deposit.
