CVC Operations

This form records any item loaned by CVC to a player, coach, or staff member, including uniforms, gear, machines, or technical equipment. It ensures all club property is tracked, maintained, and returned in the condition it was issued.

All loans are logged under the borrower’s name and verified upon return.

By submitting this form, you acknowledge and agree to the following terms and conditions:

1. Ownership: All equipment remains the property of CVC at all times. You are receiving it on temporary loan for approved, authorised purposes only.
2. Care & Responsibility: You are personally responsible for the safe use, storage, and maintenance of the item(s) while in your possession. Items must not be loaned, sold, or transferred to another person without written approval from CVC.
3. Damage, Loss, or Misuse: If equipment is damaged (beyond reasonable wear and tear), lost, or stolen, you must notify CVC immediately. Depending on the circumstances, you may be required to contribute to repair or replacement costs.
4. Return Obligations: All items must be returned by the agreed due date and in the same condition as issued. Late or non-returned items may result in administrative fees, suspension of borrowing privileges, or other disciplinary action under CVC’s policies and procedures.
5. Inspection & Verification: Upon return, CVC staff will inspect the item. Any discrepancies will be recorded and followed up through the club’s asset register and compliance process.
6. Insurance Disclaimer: CVC’s insurance covers club-owned property under specific conditions. Negligence, unauthorised use, or personal misuse is not covered and may result in personal liability for damages.
7. Indemnity & Liability: You agree to indemnify and hold harmless CVC, its officers, and volunteers from any claims, damages, or injuries arising from the use or misuse of borrowed equipment.
8. Agreement to Terms: By completing this form, you confirm that you have read and understood these terms, and agree to abide by them as a condition of borrowing CVC property.

Before proceeding with your loan request, please review the replacement value for each CVC item.

If a loaned item is lost, stolen, or returned damaged beyond reasonable wear, this value represents the cost to replace the item, which may be invoiced to you.

⚠️ If an item can be remediated (e.g., repaired, deep-cleaned, rebranded, or restored), a remediation fee may apply instead on a case-by-case basis, determined by the cost required to return the item to the condition it was in at the time of issue.
