I authorize Ambassadors Football to seek emergency medical treatment for my child if I can not be reached. I understand I am financially responsible for any medical expenses incurred.
Ambassadors Football provides transportation to and from, but not limited to - practices, games, tournaments, camps, and related program and mission aligned events when necessary. I give permission for my child to be transported in approved vehicles operated by Ambassadors staff and volunteers.
I grant permission for Ambassadors Football Pittsburgh to photograph, record, or film my child for promotional, website, fundraising, and social media purposes. Full names will not be published without additional consent.

Participation in soccer and related Ambassadors Football activities involves inherent risks including falls, collisions, physical contact, weather exposure, facility conditions, transportation risks, and other unforeseen hazards that may result in injury, illness, permanent disability, or death.

By agreeing below, I voluntarily assume all risks associated with my child’s participation in Ambassadors Football programs and activities.

I release, waive, discharge, and covenant not to sue Ambassadors Football, its directors, officers, employees, volunteers, coaches, partner organizations, league officials, facility providers, and transportation providers from any claims arising out of participation, including claims of negligence (except where prohibited by law).
