I understand that I will be contacted for follow up via email within 48 hours, in that email will be a link to the schedule. I understand to confirm the appointment I will be required to pay a $150 non refundable deposit. I acknowledge that until the deposit has been received my appointment has not been confirmed. I understand that this is a non refundable deposit. If I need to cancel or reschedule the appointment I will do this no later than 48 hours before my appointment to avoid losing my deposit.
