Ladies' Academy Registration

Parent Consent Agreement

I understand that my child has the opportunity to participate in Ladies’ Academy, an engaging after-school program designed for girls in grades 3–5. The program focuses on building confidence, leadership, positive self-image, and essential life skills through interactive activities, modeling lessons, etiquette training, and character development. I give my permission for my child to attend and participate in all Ladies’ Academy sessions, discussions, and activities. I acknowledge that this program is intended to provide a safe, supportive, and empowering environment that encourages personal growth and self-expression.

Photo and Video Release Consent

I give permission for my child to be photographed and/or recorded during Ladies’ Academy sessions, activities, and events. I understand that photos and videos may be used by Ladies’ Academy and its affiliated programs for educational, promotional, and informational purposes, including but not limited to social media, websites, brochures, and other marketing materials. I acknowledge that these images and recordings will be used respectfully to highlight the positive experiences and accomplishments of participants. I understand that no personal information about my child will be shared without additional consent.
