ACKNOWLEDGMENT OF RISK:
I understand and acknowledge that participation in activities provided by Porter House Events LLC, including but not limited to party rentals, Mini golf course, children’s craft stations, sensory play, dough-based activities, and related equipment, involves inherent risks. These risks may include, but are not limited to, personal injury, allergic reactions, choking hazards, slips, falls, property damage, or other unforeseen incidents.
I voluntarily choose to allow myself, and/or my child, and guests to participate in these activities and rentals with full knowledge of the risks involved.
ASSUMPTION OF RISK:
I knowingly and freely assume all risks, both known and unknown, associated with participation in activities and rentals provided by Porter House Events LLC, even if such risks arise from the negligence of the company, its owners, employees, contractors, or agents, to the fullest extent permitted by New Jersey law, including but not limited to, physical or psychological injury, pain, suffering, property damage, or death.
RELEASE AND WAIVER OF LIABILITY:
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Porter House Events LLC, its owners, members, employees, contractors, agents, and representatives from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to participation in these activities and/or rentals, including those resulting from ordinary negligence.
Equipment Responsibility & Supervision:
The Renter accepts full liability for any loss or damage to any and all equipment and or materials during the rental or craft station period and agrees to supervise all participants to ensure safe use.
MEDICAL AUTHORIZATION:
In the event of an emergency, I authorize Porter House Events LLC to seek medical treatment for myself and/or my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
ALLERGIES & SPECIAL CONSIDERATIONS:
I understand that craft materials may include items such as flour, food-based ingredients, dyes, scents, and small parts. I understand that candy may have allergens or may have come in contact with allergens.
INDEMNIFICATION:
I agree to indemnify and hold harmless Porter House Events LLC from any claims, damages, losses, or expenses (including attorney’s fees) arising from my, my child’s and my guests participation in the activities.
PHOTO & VIDEO RELEASE:
I grant permission for photos/videos to be used for marketing and promotional purposes.
GOVERNING LAW:
This Agreement shall be governed by and construed in accordance with the laws of the State of New Jersey.
SEVERABILITY:
If any portion of this Agreement is held invalid, the remaining portions shall continue in full force and effect.
