PLEASE FILL OUT 1 form PER child. Only fill out the form **NO MORE THAN 2 WEEKS PRIOR** to the actual birth date. Birthdays can take up to one week to process and must be picked up within 7 days of completion notice.

We are located in ERIE, PA. We serve the Erie County Area. ALL items requested MUST be picked up in our Erie, PA locations.

Special Likes/Interests (Examples: Special Needs, Cars, Dinosaurs, characters, Barbie, 18" dolls or baby dolls, artistic hobby, etc.) We TRY to include items on their wish list, but it is NOT guaranteed. We can only gift what is donated or sponsored. The more general you are, the more likely we will be able to find an item to fit the request.

I understand that I MUST pick up my items within 7 days or they will be restocked and I can not reapply for said child until the following year. I understand that I may receive misfits (gently used items) in the bundle. I agree that am in need of this birthday bundle and that the items given are free of charge.
