Member Certification Submission Form
Please use this form to upload and submit your current certification documents.

This information helps us verify qualifications and maintain accurate, up-to-date records for all coaching, judging, board, and other technical roles within our organization.
The following documents must be submitted by January 1, 2026 (as appropriate):
1. Current CBTF Coaching and Judging levels (technical members)
2. NCCP Certification (technical members)
3. Current First Aid Certificate (coaches only) - must include CPR and AED
4. Current Vulnerable Sector Check (coaches & judges over 18 years)
5. Safe Sport Certification (technical members, club chairs and ABTA board)
6. Respect in Sport Certification (technical members, club board, region board, and ABTA board
Certification Requirements

The following chart outlines which certificates are being request by role long a given certificate is valid. In this form, we will be asking you to identify the date you received any required certificates. NOTE: Should you have a certificate in a course that you are not required to submit, please submit it to ABTA as part of this process (e.g. If you are a board member who has a first aid certificate).

Respect in Sport – Important Information

ABTA now has its own designated Respect in Sport pages, which must be used when completing the course.
Please do not pay for the course — ABTA covers the cost.

Club Chairs and the ABTA Office have the access codes required to take the course at no cost.

If you need a code, please contact your Club Chair or the ABTA Office before starting.

Instructions on how to set up your Respect in Sport profile and access the course are available on the ABTA website for easy reference
Instructions:
- Ensure your documents are clear and legible.
- Accepted file formats: PDF, JPG, PNG.
- Maximum file size per upload: 5 MB.
- Submit all relevant certificates, including course completion and recertification documents.
