2026 Silver Access Application

SILVER ACCESS PROGRAM IS NOW ABLE TO ASSIST TOWARDS GOLD TIER PLANS

*Note: 1 APPLICATION PER ACTIVE POLICY*

If multiple individuals are covered under one policy, only one application is required. However, if there are different policies within a family, each policy will need a separate application.

Applications received during Open Enrollment will be processed in January to verify active policy for 2026 - You must make your first premium payment to your insurance carrier to activate your policy. Once approved, Silver Access will start making your premium payments to carrier on your behalf every month.

Applications submitted before the 15th of the month will receive approval confirmation by the end of that month. Applications submitted after the 15th will receive approval notice by the end of the following month.
