Uniacke Dog Sport Club (UDSC) Waiver and Release of Liability

Individuals who are not a member of the Uniacke Dog Sport Club (hereafter referred to as UDSC) must complete this waiver to attend any training session or event held at UDSC. For those who attend regularly to assist a member with setups or dog(s), please indicate this below on the date line.

Please ensure this waiver is completed and submitted no later than 24 hours before your rental time.

I hereby release and waive UDSC, its officers, directors, instructors, and members from any claim for damages which occur to me, my attendants, or my dog(s), or expenses arising out of any injury to any property caused by my dog(s), my attendants, or myself.

I understand that attendance at a dog training class or event is not without risk to myself, members of my family, guests who might attend, or to my dog because some of the dogs to which I will be exposed may be difficult to control even when handled by the most experienced person.

I hereby waive and release all those aforementioned from any and all liability of any nature for injury or damage which I or my dog(s) may suffer, including, specifically but without limitation, any injury or damage resulting from the action of any dog. I expressly assume the risk of such damage or injury while attending any training session or function while on the training grounds or the surrounding area.

In the event that either my dog or I are injured by another dog prior to, during, or following an event/class/seminar at the UDSC, I agree to hold harmless UDSC, its Board of Directors, officers, instructors, and property owners for any injury suffered or sustained by myself and/or my dog(s).

By signing this agreement, I acknowledge and consent that my personal information, including my name, address, phone number, and signature, may be collected and securely stored in electronic format by Uniacke Dog Sport Club (UDSC). This information will be used solely for administrative and legal purposes related to the use of the facility and will not be shared with third parties except as required by law. I understand that reasonable safeguards will be in place to protect my information.
