1. Membership Payments & Billing
• Memberships are billed monthly to the card on file.
• Billing date is the day that you sign up and will recur monthly.
• A valid card must remain on file at all times.
• All payments are final and non-refundable.
• If a payment fails, you will be invoiced immediately. Unpaid invoices suspend booking privileges until the balance is paid in full.
• Chargebacks or disputes against legitimate charges result in permanent booking bans and legal/administrative fees.
2. Cancellations & Policy Enforcement
• Memberships may be canceled only after the first billing cycle.
• Cancellations must be submitted via text to (615) 913-0742 before the next billing date to avoid future charges.
• $30 late cancellation/late reschedule fee applies for cancellations made within 24 hours of appointments.
• Missed or canceled membership payments result in loss of rollovers and benefits.
3. Membership Benefits
• Memberships include one free massage per month and discounted add-on sessions as listed at sign-up.
• Free add-ons: Hot Stones, Aromatherapy, Gua-Sha, Cupping, and more.
• Up to two family members may be added under your plan.
• Unlimited access to spa showers and sauna, with or without an appointment. (Coming soon)
• Benefits are non-transferable and non-refundable.
• Rollovers are valid only with an active membership.
4. Right to Refuse Service
Chosen Massage and Wellness reserves the right to refuse service to any client for any reason, including but not limited to: inappropriate behavior, harassment, repeated cancellations, or policy violations.
5. Legal & Agreement
• By submitting this form, you acknowledge that you have read, understood, and agreed to all terms and conditions of Chosen Massage and Wellness.
• You understand this is a legal agreement.
• All disputes will be governed by Tennessee law under Davidson County jurisdiction.
