This is a CONSULTATION form.
A consultation is required before booking to help me keep track of all appointments and deposits.
Durning your consultation we will discuss your ideas and work together to make up the perfect tattoo design for YOU.

All appointments require a $80 cash deposit BEFORE booking which will be collected during your consultation.
After we yap and your deposit has been collected we will book the time and date that works for you!
