  • I understand that my child is enrolling in Fern and Fir a flexible, individualized learning program designed to support growth, community, and creativity. / recognize that the program includes a variety of academic, enrichment, and physical activities, both indoors and outdoors.

    I understand that participation in any program involving children carries inherent risks, including (but not limited to) accidents, minor injuries, or unexpected behaviors. I accept full responsibility for these risks on behalf of my child.

    By signing this agreement, / release and hold harmless Fern and Fir, its founders, staff, volunteers, and partners from any claims, liabilities, damages, or expenses that may arise from my child's participation, except in cases of gross negligence or intentional misconduct.

    In the event of an emergency, / authorize Victoria Demarest to seek necessary medical care for my child. / understand that I am responsible for any related medical expenses. This might include calling an ambulance, at my expense, if an emergency occurs. / understand that in such cases, Victoria Demarest will notify me right away, or the emergency contact provided.

    I understand that deposits and tuition payments secure my child's spot in the program. Deposits are non-refundable unless otherwise stated. Tuition must be paid in full by the agreed-upon due date to maintain enrollment. / understand that all sales are final/non-refundable, unless Victoria Demarest has to cancel due to personal reasons or cannot provide a make-up day or sub.

  • I understand that Fern and Fir is a collaborative learning environment. / agree to support my child in following program guidelines, respecting others, and engaging in a way that contributes positively to the community.

  • Child Illness - I understand that I will be required that my child(ren) be kept out of child care when ill or otherwise unable to participate in the daily activities of the center (see information below

    Medication- I authorize Victoria Demarest to administer medication (both prescription and non-prescription) in accordance with the written instructions I will provide. Additionally, I understand that all medication must be in its original containers and that prescription medication must have my child's name on the bottle.

    Ointments and Creams- I authorize Victoria Demarest to administer ointment and creams to my child as needed, which / will provide in the original container with their name on it.

    Transportation- I authorize Victoria Demarest to transport my child in her personal vehicle if there is a medical emergency or extreme weather conditions, if I or provided emergency contacts aren't available.

     

    Parent(s)/ Guardian(s) hereby certify that we provide medical insurance coverage for my , and assume all financial responsibility for seeking medical attention and treatment in the case that an incident occurs with

    Occasionally accidents may occur during a child's care (falls, trips, etc) that may result in medical attention. We agree that neither Victoria Demarest or any of the administration at the Church shall be held responsible for any injuries that may occur during the contractual agreement of child care. This applies in instances of accident and/or injury outside the scope of negligence.

  • I give my permission for my child to go on field trips with Victoria Demarest I understand that these trips are supervised in accordance with the Office of Child Care rules and regulations. I also understand that these trips include potential transportation in which the parents will know of and agree to ahead of time depending on field trips.

     

  • Fern and Fir follows the Illness Policy given by the Office of Child Care. However, at times, we may need to use discretion beyond the scope of the policy to ensure the health and safety of all our children, families, and staff that we serve.

    Children who exhibit any of the following symptoms will be sent home/should not be brought to school.

    Temperature of 100.4 degrees and shows signs of being extraordinarily clinging, demanding to be held constantly, unusual lethargy, etc. Severe or Persistent cough with a runny nose Contagious disease: Staph, Strep, Chicken Pox, Measles, Pink Eye, etc. (these require a doctor's note upon returning to school) Diarrhea or Vomiting Unusual yellow color to skin or eyes Skin or eye lesions, or rashes that are severe, weeping or pus-filled (if we are in doubt about a rash of unknown origins, we will ask that you pick up your child until they are cleared by a doctor or are rash-free) Difficulty breathing or abnormal wheezing Complaints of severe pain Or is diagnosed as having a day-care restricted disease per Health Division Rule 333-19-200 If a child displays any symptom of illness, especially those listed above, they need to be symptom-free for 24 hours before returning to school. If a child is sent home due to one or more of the above symptoms, they should not return to school the following day. I understand the above Illness Policy and will do my best to uphold it. Furthermore, I understand that if my child is sick/displays any of the above symptoms, I will be notified and have one hour to pick up my child/have my child picked up before late fees apply.
