DOCUMENT CHECKLIST
ALL TAX FILERS MUST PROVIDE THE FOLLOWING COPY:
VALID GOVERNMENT ID
SOCIAL SECURITY CARD
(A previous tax return can be used instead of a social security card)
PROOF OF ALL INCOME STATEMENTS
(Form W-2, 1099s, Unemployment Form, Schedule C, Expense sheet signed for self-employed, Interest and Dividends, etc.)
Additional Documents and Requirements For the Following:
Head of Household
A. Must pay more than 50% to pay home.
B. Mortgage statement, lease, utility bills, property taxes, grocery receipts.
C. Dependent letter from the school or medical record from a provider showing the same address.
Note: You are not required to provide all the information above. “A” is required and he/she can choose between “B” and “C”. If client can not provide any information above he/she is not eligible to file as HOH instead client will be files as “single with dependent”.
Earned Income Credit
A. Social Security cards, a Social Security number verification letter, or other U.S. government identification for all persons listed on the return.
B. Birth certificates & marriage certificates if related by marriage, or notarized relationship affidavit.
C. School letter for proof of enrollment, health insurance card, or medical statement showing address.
D. Proof of earned income, ex: W2, 1099’S, Schedule C expense sheet signed for self-employed, Interest, dividends, etc.
E. Dependents must meet age, relationship, and residency test Note: Need (1) document from A,B,C & D. “E” is required.
Child Tax Credit or ACT (One document from below)
A. Birth certificate or marriage certificate if related by marriage, or relationship affidavit. B. The same documents used for the Head of Household and Earned Income Credit.
C. Childcare letter with Ein, W10 form, https://www.irs.gov/pub/irs-pdf/fw10.pdf, or the child caregiver's social security number and address.
D. If the dependent is disabled proof of disability, like an SSA award letter or letter from a doctor.
E. Form 6419 from IRS stating how much CTC was received and for who, or login into the IRS CTC portal.
AOTC Credit (Full-time Student)
A. 1098-T
B. Must not have claimed the credit for over 4 years.
Household Employee
A. Notarized Affidavit
B. Description of income and name and phone number of employer.
(Remember taxpayers can’t make over $10,000 for the year nor more than $2,000 per household) Self-employed (Schedule C)
A. Proof of expenses, ledgers, receipts, log book, bank statements, flyers, or signed Schedule C expense sheet.
B. EIN, or DBA document from their city if using their social security number, business license, etc,
Disabled Dependent
A. Must be a qualifying child or qualifying relative.
B. Must have a disability letter or doctor's statement.
