I understand that participation in the Connecticut Hurricanes Drum & Bugle Corps camp involves physical activity. I acknowledge that there are inherent risks associated with these activities, and I voluntarily assume all such risks on behalf of myself / my child.

I certify that I / my child am physically able to participate and that I have disclosed any relevant medical conditions above. In the event of illness or injury, I authorize the administration, staff, or any attending medical personnel to provide necessary medical treatment, including first aid, emergency transport, or hospital care, as deemed appropriate.

I release and hold harmless the Connecticut Hurricanes Drum & Bugle Corps, its staff, volunteers, and associated facilities from any and all claims of injury, loss, or damage arising from participation in this event, except in cases of gross negligence or willful misconduct.
