    Founded in 1913 on the campus of Howard University in Washington, D.C., Delta Sigma Theta Sorority, Incorporated, is a public service, nonprofit organization whose purpose is to provide services and programs to promote human welfare and assistance for persons in need. The membership of more than 350,000 predominantly African-American college-educated women has more than 1,000 undergraduate and alumnae chapters located worldwide. Delta’s extensive Five-Point Program focuses on five key areas: economic development, educational development, international awareness and involvement, physical and mental health, and political awareness and involvement. The national headquarters is located in Washington, D.C.

    Morristown Alumnae Chapter was chartered in 1987. The membership maintains active engagement in the community by implementing initiatives that support the organization’s Five-Point Programmatic Thrust and theme: “Delta Sigma Theta – A Sisterhood Called to Serve: Transforming Lives, Impacting Communities.”

    Students eligible for this scholarship must:

    • Be a high school senior who is African American or of African descent and at least one of the following:
      • Resides in Morris, Sussex, Warren or Hunterdon county
      • Attends high school in Morris, Sussex, Warren or Hunterdon county
      • Actively participates in one of Morristown Alumnae Chapter’s youth programs
    • Have a minimum cumulative GPA of 2.75 on a 4.0 point scale
    • Have been accepted to an accredited two-year or four-year college or university
    • Agree to be interviewed by the Scholarship Committee as part of the selection process

    Application Procedures:

    APPLICATION
    This online application must be completed and submitted by the application deadline to be considered. Offline applications will not be accepted. 

    ESSAY
    Compose and submit your personal essay. The essay must be submitted as part of the online application. Offline submissions of the essay will not be accepted and will render the application incomplete.

    REQUIRED SUPPORTING DOCUMENTS
    The following documents are required as part of your application and must be received by the deadline. These Official Transcript and Letter of Recommendation may be uploaded with your application, uploaded separately by the school or provider at https://form.jotform.com/253238551813154, and/or sent via e-mail or U.S. mail to the address listed below:

    • Official Transcript - An official high school transcript of grades covering all years of high school with cumulative GPA. 
    • Letter of Recommendation - One (1) letter of recommendation. Letter must be from a school administrator or faculty member (e.g., teacher, guidance counselor, principal, coach, etc.) and printed on school letterhead. The school's letterhead must appear at the top or bottom of the letter and must include the school's logo and contact information.
    • Verification of Community Service - Using the Community Service Verification Form provide details for an organization where you volunteered. Include the name and purpose of the organization along with how you contributed to the organization's mission. Describe the specific activities you performed as a volunteer, the responsibilities you handled, the skills you used, and any meaningful impact your volunteering provided. Once submitted the form will be forward to the organization for validation and final submission.

    TIMELINE

    PHOTOGRAPH, MEDIA, AND VIDEO AUTHORIZATION RELEASE FORM

    I/We, (“Parent/Guardian”), as parent(s) or legal guardian(s) of {fullName122}, give permission for the Morristown Alumnae Chapter of Delta Sigma Theta Sorority, Incorporated (the “Chapter”) to publish on the Internet or media still photographs or moving images, including, if applicable any sound recordings accompanying the images (“Images”) taken of my child in connection with the award of a scholarship from the Chapter, without payment or any consideration and without notifying me in advance.

    I/We also give permission for the Chapter to highlight my child’s achievements and activities in efforts to promote its Scholarship Program through newspapers, radio, TV, the web, DVDs, displays, brochures, social media and other types of media without payment or any consideration and without notifying me.

    I/We understand and agree that these Images will become the property of the Chapter, which shall have complete ownership of the Images. I hereby irrevocably authorized the Chapter to publish or distribute these Images for the purpose of publicizing the Chapter’s programs, including its Scholarship Program or for any other lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my child’s likeness appears. Additionally, I waive any rights to royalties or other compensation arising out of or related to the use of the Images.

    I/We hereby hold harmless and release and forever discharge the Chapter and any of its officers and members; Delta Sigma Theta Sorority, Incorporated; its officers; National Executive Board; employees; members; representatives; agents; and assigns from any and all claims, costs, suits, actions, judgments, and expenses which my child, his/her heirs, representatives, executors, administrators, or any other persons acting on his/her behalf have or may have by reason of the use of the Images. This release specifically includes, without limitation, a complete release and discharge of any liability by virtue of any editing, distortion, alteration, or optical illusion, whether intentional or otherwise, that may occur or be produced in the taking of or editing of said Images, unless it can be shown that such was maliciously caused, produced and published solely for the purpose of subjecting my child to conspicuous ridicule, scandal, reproach, scorn and indignity.

    I/We hereby certify that I/we are the parents/guardians of {fullName122}, authorized legally to give this consent, and do hereby give my/our consent without reservation to the foregoing on behalf of my/our child.
