the parents of the above participant, who is registering as a youth athlete with CTX Flames track club, hereby give my approval for him/her to participate in any/all activities during the current season.
will respect and obey all laws and the athlete's Code of Conduct of AAU and USATF. (Please see code of conduct tab in menu before agreeing).
further hereby release, absolve, indemnify, and hold harmless the CTX Flames track club, organizers, coaches, or any supervisors appointed by them.
assume all risk and hazards incidental to the conduct of the activities and transportation to and from activities
