Please read this agreement carefully before submitting your intake form. Once submitted, this form serves as your formal engagement with BDreamss LLC for tax preparation services for the current tax year or any prior years you’ve selected.

🔁 Retraction Policy

Submitting this form initiates a formal relationship with our firm. If you choose not to file your return through BDreamss LLC after your return has been completed, a $50 Retraction Fee will be assessed and invoiced. This fee compensates our team for time spent reviewing your intake, verifying documents, entering your data, and preparing your tax return even if it was never filed.

Please understand:Sending personal information to multiple tax companies at once is strongly discouraged. This not only increases your identity theft risk, but it also signals a lack of commitment to the professional you engaged. We operate as a firm, and our preparers' time and expertise deserve to be respected.

By continuing beyond this point, you are confirming that you are ready to move forward with BDreamss LLC as your tax provider for this filing year.

 Consent to Use and Disclose Information 

By signing, you authorize BDreamss LLC to collect, store, and use your personal and financial information for the purpose of:

• Processing payments, scheduling, and communications;
• Sharing limited data with trusted partners or processors when necessary for service delivery.

 BDreamss LLC will not sell, rent, or disclose your information for any other purpose without additional written consent.

• Refund advances are optional. If you do not wish to be considered for an advance, simply indicate that in the form.

Once this form is submitted, your next step is to upload all required documents in your secure TaxSlayer client portal or intake form. This includes:

• W-2s, 1099s, or any income documentation

Failure to provide complete documentation will delay your tax preparation.

📌 Final Acknowledgment

Our fee for the preparation of your tax returns will be due and payable upon presentation of your completed income tax filing. It is company policy not to release tax returns or any tax related reports, schedules, information, advice, or notes without payment in full. Should a situation arise where services have been rendered but payment is not received and our firm is forced to or elects to seek legal assistance to collect fees due to us, please note that you may be asked to reimburse our firm for the legal costs to collect any outstanding balance due.

 Because we understand that cost is an important issue for many clients, we will do our best to provide you with an estimate of our fees prior to completing your returns but please understand that circumstances arise which may raise or lower the estimated fee. We will contact you if such a situation arises after you receive an estimate.

• You are ready to move forward with tax preparation through BDreamss LLC 
• You understand that failure to file after return preparation will result in a $100 retraction fee
• You accept full responsibility for providing truthful and complete information
• I confirm that I have read, understand, and agree to all terms outlined in this Intake Agreement & Disclosure.
• I provide consent for payment processing, service delivery, and information use as described above.
• By signing this agreement, you acknowledge that all information provided to BDreamss LLC is accurate and complete. Services will be performed based on this information. Submitting false, misleading, or incomplete details may result in cancellation of services without refund.
