Consent for off-lead walking.

Dogs will only be walked off-lead with consent from the owner and once recall has been established. Even when consent has been given it will not be guaranteed to walk your dog off-lead as it will depends on other factors such as location and whether safe to do so.

By signing this for and consenting you are giving permission for Jade Lyons (Tails That Wag) to walk your dog off-lead and you agree that you understand potential consequences that can occur to yours or any dog. You will be accepting full liability for any damaged caused to or by your dog and take full responsibility for the actions or disappearance of you rdog while off-lead walking.

Consent for Veterinary Care
If anything should happen to your dog while in the care of Tails That Wag permission is needed to take your dog either to a vet you are registered at or, in case of emergency, a local vet.

A call will be made to you if any signs of sickness are noticed upon visiting or during a walk so you can decide next steps, however should we not be able to make contact and it’s decided your dog needs veterinary care we need your consent to do this.

By signing this you agree that Tails That Wag can take your dog to the Vet in the absence of your presence and you take full responsibility for the payment of the visit.

Key Holder Consent

Access to the home will be needed to collect and drop your dog back for their walk so it will be necessary to either have a key left, at owners risk, or for a copy of the key to be given to Tails That Wag.

By signing you agree that if a key will be left this will be your own responsibility and not that of Tails That Wag, should anything happen to the key. Should a spare key be given it will be the responsibility of Jade Lyons (Tails That Wag), and you agree for access to be given to the home to collect your dog.
