IMAGE USE AUTHORIZATION & LIABILITY RELEASE
I (or my parent/legal guardian if I am under 18 years of age) hereby agree to the following:
IMAGE & MEDIA AUTHORIZATION
I grant 9SenceMaison(“the Studio”), including its owners, instructors, employees, contractors, agents, and assigns, the irrevocable, perpetual, worldwide right to photograph, film, or otherwise record me during in-person classes, workshops, or events.
I authorize 9SenceMaison to use, reproduce, edit, publish, distribute, and display such images, audio, or video recordings in any medium, including but not limited to websites, social media, marketing materials, advertisements, and educational content, without compensation or further approval.
I waive any right to inspect or approve the final materials or the manner in which they are used.
ASSUMPTION OF RISK & LIABILITY RELEASE
I understand that participation in in-person yoga and movement practices involves inherent risks, including but not limited to:
•Physical injury, strain, or aggravation of existing conditions
•Slips, trips, or falls on floors, mats, or props
•Contact with other participants or shared equipment
•Exposure to illness
•Emotional or psychological discomfort
I voluntarily assume all such risks.
I acknowledge that instructors are not medical professionals, and that classes are not a substitute for medical care, diagnosis, or treatment. I confirm that I am in good physical and mental health to participate, have consulted a physician if necessary, and agree to modify or stop participation if I experience pain, discomfort, or distress.
RELEASE, WAIVER & INDEMNIFICATION
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless the Studio and its affiliates from any and all claims, liabilities, losses, damages, or expenses arising out of my participation or use of images, including those resulting from negligence.
I further agree to indemnify and defend the Studio against any third-party claims arising from my participation.
This agreement applies to all current and future classes and events and shall be governed by the laws of the State of California.
I understand that this is a legally binding agreement and that by signing below, I am giving up certain legal rights.
